Celecoxib should not be used in:Hypersensitive to the active ingredient and sulfonamides (active against bacteria)
Patients after taking aspirin (non-opioid painkillers) or other NSAIDs showed allergic reactions such as asthma, acute rhinitis, nasal polyps, mucosal swelling and hives
Heart disease due to narrowing of the coronary arteries, moderate to severe heart failure (NYHA stages II - IV.)
calcification of brain blood vessels
Stomach ulcers or ulcers in stomach bleeding or intestinal bleeding and in inflammatory bowel disease
severe hepatic impairment
severe kidney damage.
In addition, women should avoid in childbearing age without safe contraception protection on the use of celecoxib.
Only after strict medical benefit-risk assessment should be administered celecoxib with
significant risk factors for the occurrence of cardiovascular disease
significant risk factors for the development of disorders in stomach and intestines
Combination with anticoagulant drugs (anticoagulants).
Elderly patients should receive careful monitoring physician during treatment with celecoxib.
Some people have a deficiency of the enzyme CYP2C9 which is necessary for the degradation of celecoxib. The Blutonzentrationen of celecoxib are probably markedly increased in such patients. If the enzyme deficiency known, individuals should be treated with medical care.
What should I watch for pregnancy and breastfeeding?
There are no clinical data on application risks of celecoxib in pregnant women. However, malformations were detected in studies on rats. The potential risk for humans during pregnancy is unknown but can not be excluded. Moreover, celecoxib may cause an inhibition of uterine contractions. Celecoxib is therefore in pregnancy not apply. Cyclosporin and tacrolimus, immunologicals, which are used in autoimmune diseases, are amplified in its nephrotoxic effect.
With simultaneous use of low-dose acetylsalicylic acid (a non-opioid analgesics) creates an increased risk of complications from ulcers.
Celecoxib should not be administered with ACE inhibitors and Angiosin-II inhibitors (both groups of anti-hypertensives). The combination with these cardiovascular-active substances can lead to acute renal failure.
Basically antihypertensive agents (antihypertensives) and dehydrating agents (diuretics) with celecoxib can be reduced in their effect.
During celecoxib treatment, the blood levels of dextromethorphan soar, an antitussive drug. The effect of the agent may be enhanced cough. The same applies to drugs for depression (antidepressants), neuroleptics and other psychotropic drugs such as citalopram, imipramine and diazepam and agents against cardiac arrhythmias.
Fluconazole, an anti-fungus diseases, causes an increase of celecoxib concentration in the blood and can thus also result in an increased effect.
Rifampicin (tuberculosis agent), however, may antagonize the effects of celecoxib. The same applies to carbamazepine (an antiepileptic drug), and barbiturates (sedatives).
The combination of methotrexate, a Immunologikum, celecoxib should be monitored medically appropriate. Although lithium, a drug for depression, is administered the same time, careful medical supervision is necessary.
