Active substance: Metoprolol
Metoprolol is a drug from the group of selective β1-adrenergic receptor blocker (beta-blockers) and is used to treat high blood pressure, coronary heart disease, use of cardiac arrhythmias and for acute treatment of myocardial infarction. Another field of application is the prophylaxis of migraine.
Therapeutic indications (indications)
Metoprolol can be used in the treatment of myocardial infarction, hypertension, coronary heart disease, heart failure and certain cardiac arrhythmias. In addition, metoprolol is used in migraine patients for seizure prophylaxis, if certain criteria are met.
Contraindications (contraindications)
Metoprolol is contraindicated in congestive heart failure (NYHA IV), AV block 2nd or 3rd degree, bradycardia (resting heart rate less than 50 beats per minute prior to treatment), hypotension (blood pressure systolic less than 90 mmHg) and bronchial hyperreactivity (eg in connection with, inter alia, bronchial asthma) are applied. In patients with peripheral circulatory disorders, a gain of discomfort is possible.
Interactions with other drugs
Intravenous administration of metoprolol in patients who already received treatment with calcium antagonists (verapamil and diltiazem from) or other antiarrhythmic drugs (such as disopyramide) can cause severe bradycardia arrhythmias and is therefore contraindicated; as the i.v. administration of the aforementioned calcium channel blockers and antiarrhythmic drugs during treatment with metoprolol (except intensive care).
Use during pregnancy and lactation
Metoprolol should be used during pregnancy and lactation only under strict medical supervision. In animals (mice and rats), there was evidence of hypoperfusion of the placenta and consequently fetal growth retardation. Also, the risk of childhood arrhythmias can not be excluded.
Special populations (diabetic kidney disease)
With simultaneous use of metoprolol and insulin or oral hypoglycemic agents whose effect may be enhanced or extended, which increases the risk of hypoglycemia. At the same warning signs of hypoglycemia (tachycardia and tremor) are concealed or mitigated. Therefore, regular blood sugar checks are required in diabetic patients receiving therapy with metoprolol.
Adverse reactions (side effects)
Besides occasional hypersensitivity reactions related side effects such as ringing in the ears and dizziness can be observed through the reduction in blood pressure. More rarely occur CNS changes (fatigue, hallucinations), bronchospasm, potency and Harnentleerungsstörungen. The capability of making active use on the road, to operate machinery or to engage in potentially hazardous can be affected by metoprolol. Metoprolol strengthens further the effects of other antihypertensive drugs (indication of the severe drops in blood pressure with the use of β-blockers with calcium channel blockers of the verapamil and diltiazem type). Also effective reinforcing effects with other heart rhythm affecting drugs are known. Beta-blockers can activate psoriasis in some cases.
Overdose
The extent of poisoning depends on the supplied amount of substance and manifests itself in the form of severe falls in blood pressure, low heart rate up to cardiac arrest with corresponding functional loss of organs. An overdose requires intensive medical treatment and monitoring.
